Wellness Funds support projects by non-profit groups working to improve health in their communities and address the health priorities identified by the Community Health Boards (CHBs) in their current community health plans.  Over the past decade, there is an increasing focus on a population health-based approach to help prevent illness. This model aims to maintain and improve the health of the population by addressing those aspects of our lives that determine our health, and by improving the communities in which they live. 

Wellness Funds:

• Benefit local community  

• Advance community health board priorities using a population health-based approach to address the social determinants of health

• Encourage partnerships through relationship building and collaboration

• Provide start-up or bridge-funding – not intended to be an ongoing source of funding
